Information submitted through a general contact form or ad lead form before you become a patient is not considered PHI until you are an established patient of the Practice.

These vendors are contractually bound to protect your information and, where applicable, to comply with HIPAA as our business associates. We do not sell your personal information to third parties.

6. Data Security

We use reasonable administrative, technical, and physical safeguards to protect information collected through the Site and in our clinical systems. No method of electronic transmission or storage is 100% secure, and we cannot guarantee absolute security.

7. Children’s Privacy

Our Site is not directed to children under 13, and we do not knowingly collect personal information from children through the Site without parental involvement.
